HER2 Lung Cancer Explained Simply: What Makes It Different?

What exactly is HER2, and why does it matter?
As a thoracic medical oncologist, this is one of the most common questions I hear in clinic. Learning what makes this type of lung cancer unique can help you better understand your diagnosis and the treatment options your cancer care team may discuss with you.
How HER2 mutations affect cancer cell growth
Think of every cancer cell as carrying its own "instruction manual." These instructions tell the cell when to grow, divide, and survive. In some lung cancers, a change (called a mutation or actionable genomic alteration) occurs in the HER2 gene (also known as ERBB2). This mutation causes the HER2 protein to send continuous signals that tell the cancer cell to grow and survive.
How common is HER2-positive lung cancer?
Although HER2 is best known in breast cancer, it can also occur in non-small cell lung cancer (NSCLC), most commonly in adenocarcinoma. HER2 alterations are relatively uncommon, occurring in approximately 2 to 4 percent of lung cancers.1 However, identifying them is incredibly important because they represent a distinct molecular subtype with an expanding number of treatment options.
Understanding the different types of HER2 alterations
One area that often causes confusion is that not all HER2 alterations are the same. HER2 mutations, HER2 amplification, and HER2 overexpression are distinct biological changes within the cancer cell. Although the names sound similar, they are not interchangeable. These differences can influence which treatments have been studied and may be considered for a particular patient. That's why comprehensive biomarker testing and careful interpretation of those results are a critical part of caring for someone with advanced lung cancer.
Who is more likely to have HER2-positive lung cancer?
Unlike some other lung cancers, HER2-positive disease is often diagnosed in people with little or no smoking history, although it can occur in anyone. It also tends to affect younger patients more frequently than many other forms of lung cancer and has a greater tendency to spread to the brain, which is one reason brain imaging is an important part of care. None of these characteristics, however, define an individual person's experience. Every patient's journey is unique.
Why biomarker testing matters for HER2 lung cancer
One of the most important advances in lung cancer over the past decade has been our ability to move beyond treating cancer based only on where it started. Today, we increasingly treat lung cancer based on its "molecular fingerprint," e.g. the specific genetic changes driving the cancer's growth. HER2 is one of those important molecular drivers.
This is why your healthcare team orders next-generation sequencing (NGS) or comprehensive biomarker testing. Rather than making treatment decisions based solely on what the cancer looks like under a microscope, these tests help identify whether the tumor carries an actionable genetic alteration, like HER2.2
New treatment options and ongoing research
The encouraging news is that research in HER2-positive lung cancer has accelerated dramatically. Just a few years ago, treatment options were limited. Today, several HER2-directed therapies have become available, and even more are being studied in clinical trials. Researchers are also working to better understand why some treatments work exceptionally well for certain patients and how to overcome resistance when cancers eventually adapt.
For patients and families, all of this progress represents something incredibly meaningful: more options and more hope.
